ABSTRACT

Insights on the indirect effects of the COVID-19 pandemic are critical for designing and implementing policies to alleviate the food security burden it may have caused, and for bolstering rural communities against similar macroeconomic shocks in the future. Yet estimating the causal effects of the pandemic is difficult due to its ubiquitous nature and entanglement with other shocks. In this descriptive study, we combine high-resolution satellite imagery to control for plot-level rainfall with household socio-economic panel data from 2014, 2016, 2019 and 2020, to differentiate the effect of the pandemic from climatic shocks on food security in Morogoro, Tanzania. We find evidence of decreased incomes, increased prices of staple foods, and increased food insecurity in 2020 relative to previous years, and link these changes to the pandemic by asking households about their perceptions of COVID-19. Respondents overwhelmingly attribute economic hardships to the pandemic, with perceived impacts differing by asset level.

ABSTRACT

The emergence of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variant Omicron (B.1.1.529) has been a cause for serious concern worldwide due to its high rate of transmission and number of mutations. During genomic studies in South Africa and Botswana in November 2021, a new variant of SARS-CoV-2 was identified associated with a rapid resurgence of infections in Gauteng Province, South Africa. Omicron variant was identified as a type of concern by the World Health Organization after sequence uploads of the first genome. It was subsequently identified in 87 countries within three weeks. The Omicron variant is a very exceptional virus carrying more than 30 mutations in the spike glycoprotein that are predicted to affect antibody neutralization and spike function. Omicron is highly contagious and spreads faster than previous variants, but may cause less severe symptoms than previous variants. Omicron variant can evade the immune system. It can also evade the vaccine responses developed against COVID-19. Rapid and careful preventive steps, including vaccination, will always be a key for suppression of the Omicron variant. This review summarizes the highly mutated regions, core infectiousness, vaccine elimination, and antibody resistance of the Omicron variant of SARSCoV-2.

ABSTRACT

The severity of COVID-19 disease varies substantially between individuals, with some infections being asymptomatic while others are fatal. Several risk factors have been identified that affect the progression of SARS-CoV-2 to severe COVID-19. They include age, smoking and presence of underlying comorbidities such as respiratory illness, HIV, anemia and obesity. Given that respiratory illness is one such comorbidity and is affected by hand hygiene, it is plausible that improving access to hand washing could lower the risk of severe COVID-19 among a population. In this paper, we estimate the potential impact of improved access to hand washing on the risk of respiratory illness and its knock-on impact on the risk of developing severe COVID-19 disease across Zimbabwe. We use a geospatial model that allows us to estimate differential clinical risk at the district level. Results show that the current risk of severe disease is heterogeneous across the country, due to differences in individual characteristics and household conditions. This study demonstrates how household level improved access to handwashing could lead to reductions in the risk of severe COVID-19 of up to 16% from the estimated current levels across all districts. Taken alongside the likely impact on transmission of SARS-CoV-2 itself, as well as countless other pathogens, this result adds further support for the expansion of access to hand washing across the country. It also highlights the spatial differences in risk of severe COVID-19, and thus the opportunity for better planning to focus limited resources in high risk areas in order to potentially reduce the number of severe cases.

ABSTRACT

Objective: To assess the global epidemic of Coronavirus disease 2019 (COVID-19) in January 2023 and the risk of importation. Methods: According to the daily COVID-19 data publicly released by Johns Hopkins University, combined with the travel restrictions published by Sherpa, an epidemiological description method was used to provide a comprehensive and timely assessment of the global epidemic risk through a general overview, a comprehensive assessment of the epidemic trends in each continent and key countries, as well as a comprehensive analysis of the epidemic and travel requirements in 14 neighbouring countries. Results: Compared with the previous month, the number of confirmed cases and deaths respectively decreased by 40.37% and increased by 147.95% globally in January 2023. Daily new confirmed cases showed a decreasing trend, while deaths in all continents stayed stable except Asia in January. The time taken for every 50 million new confirmed cases and 500 thousand deaths globally has increased slightly. The number of deaths in Japan, America, China, and Australia increased rapidly. Zambia, Argentina, New Zealand and Mexico were the countries that showed a sharp rise in the number of deaths in January. Russia was the country with special concern among the 14 neighbouring countries. Conclusion: The global daily new confirmed cases and deaths showed a downward trend in January, but the epidemic situation was prominent in some regions and countries. It is necessary to continuously monitor countries with global focus and establish a communication mechanism with relevant agencies to exchange information and provide timely warnings.

ABSTRACT

Based on a case study in each country, this study documents the views of Mozambican and Tanzanian smallholders regarding Chinese agricultural investments and the extent to which investors abide by their legitimate land tenure rights as defined by the Voluntary Guidelines for the Responsible Governance of Tenure of Land, Forests and Fisheries in the Context of National Food Security (VGGTs). The VGGTs offer guidelines to government on how to protect the land tenure of rural communities when land is being acquired for large-scale land investments. The study also assessed the impact of the COVID-19 pandemic on smallholders. Due to COVID-19, instead of fieldwork, we conducted telephone interviews with 20 smallholders in Mozambique and 35 in Tanzania. The Mozambican case showed that even when land set aside for investors was not in dispute, smallholders still had unmet expectations, especially regarding investors' corporate social responsibility activities. In the Tanzanian case, even though the land leased by the Chinese investor had been designated as general land, it had laid fallow for a long period, and smallholders had moved back onto the land, only to be displaced in 2017. Although smallholders' views on the investment were mixed, the case underscored the need for government to assess current land use before allocating it to investors - regardless of how the land is classified and especially in areas where land shortages are creating conflict. The cases show that even if communities are consulted about proposed land investments, guidelines need to include clauses that allow for ongoing communications between investors, communities and government officials such that if communities are unsatisfied with the results of the investment, renegotiation is possible. Further, in the event of crises, such as COVID-19, investors should partner with communities and government to limit the extent of harm in communities as a result of the crisis.

ABSTRACT

This paper assesses the influence of e-commerce service quality on domestic tourists' satisfaction in Tanzania. This was important following closure of international tourism amid COVID-19. A multiple regression analysis to data collected from 150 local tourists in Dar es Salaam assessed influence of variables (reliability of e-services, perceived transaction security, responsiveness of e-services, and e-service ease of use) on customer satisfaction. Findings reveal variables' positive and significant influence statistically at p < 0.05. This implies that domestic tourists' satisfaction through e-commerce service quality in Tanzania is facilitated by reliability of e-services, perceived transaction security, responsiveness of e-services and e-service ease of use. Tourism managers need to ensure that all important pre-requisites necessary for tourism e-service quality provision are in place. The government and other responsible stakeholders need to invest in facilitative digital facilities to ensure optimal e-service quality are reliable at all times.

ABSTRACT

In December 2022 (from 00: 00, 1 December to 24: 00, 31 December), except COVID-19, a total of 278 907 cases of notifiable communicable diseases, including 2 384 deaths, were reported in China (except Hong Kong, Macao Special Administrative Regions and Taiwan Province, the same below). In communicable diseases in class A, no cases and no deaths were reported. In communicable diseases in class B, no cases and no deaths of severe acute respiratory syndrome, poliomyelitis, human infection with highly pathogenic avian influenza virus, diphtheria and human infection with avian influenza A (H7N9) virus were reported. For the remaining 21 communicable diseases except COVID-19 in class B, a total of 148 573 cases were reported, a decrease of 29% compared with last month (210 178 cases) and a decrease of 46% compared with the same period in 2021 (273 361 cases). The first 5 diseases in terms of reported case number were viral hepatitis, pulmonary tuberculosis, syphilis, gonorrhea and AIDS, accounting for 96% of the total reported cases in class B. A total of 2 384 deaths were reported, an increase of 28% (521 deaths) compared with last month (1 863 deaths) and a decrease of 14% compared with the same period in 2021 (2 763 deaths). In class C communicable diseases, a total of 130 334 cases were reported, a decrease of 33% compared with last month (195 072 cases) and a decrease of 69% compared with the same period in 2021 (420 015 cases). The first 3 diseases in terms of reported case number were influenza, other infectious diarrhea, and hand foot and mouth disease (HFMD), accounting for 96% of the total reported cases in class C. Compared with last month, except filariasis which had no incidence, the reported case number of leprosy was same, and the disease with reported case increase was echinococcosis (47 cases, 48%), the reported cases of other diseases all decreased, the first 3 diseases with reported case decreases were HFMD (22 886 cases, 45%), other infectious diarrhea (21 962 cases, 43%) and influenza (14 775 cases, 18%). Compared with the same period in 2021, except filariasis which had no incidence, the reported cases of other diseases all decreased, the first 3 diseases with reported case decreases were influenza (181 158 cases, 73%), other infectious diarrhea (53 502 cases, 65%) and HFMD (46 674 cases, 63%). No death caused by class C communicable disease was reported, same to last month and a decrease of 7 deaths compared with the same period in 2021 (7 deaths).

ABSTRACT

Malaria is an endemic disease in a true sense. It is an acute febrile disease caused due to the parasite Plasmodium. However, unlike COVID-19, it failed to raise an international concern or gain the scientific limelight. Most of the 200 million globally affected by malaria, half of them are from Africa. Four of the nations, Nigeria (25%), the Democratic Republic of the Congo (11%), Mozambique (5%), and Uganda (4%), account for half of the world's malaria burden and is the leading cause of illness and death. In 2019, an estimated 5-6 million people died of malaria - most of them are young children in sub-Saharan Africa. Many of the countries affected by malaria have the lowest economic status. In the malaria-endemic region, the most vulnerable groups are young children and pregnant women. The costs of malaria are enormous to individuals, families, communities, societies, and nations. After a struggle for three decades, the much-awaited malaria vaccine, RTS, S (brand name Mosquirix), was finally launched;but it came with its controversies and allegations. This review explored the different angles of this disease, the vaccine development, and the emerging debates.

ABSTRACT

In Africa, there is frequently an extreme need for nurses and other healthcare professionals. Overcrowding, a lack of administrative oversight, and a lack of resources are all problems in health settings. Health professionals may develop psychological damage referred to as "insufficient resource trauma" as a result of these difficulties, which can lower morale and motivation. Such trauma reduces the standard of treatment provided and causes professional "burn-out," a problem that has become more significant in the wake of the SARS-CoV-2 pandemic. One of the nurses who received training in PSBH-N in 2006 was promoted to the position of MoH Nursing Director by 2019. This nurse reflected on her personal experience and said, "The nurses trained in PSBH are among the top nurses in the country today," when she learned of the new PSBH training for MoH QAU employees. In order to train all 877 registered nurses and 373 licensed practical nurses (LPNs) in the nation during the following three to five years, the director requested that LeBoHA host PSBH-N workshops. The countrywide rollout's training and evaluation are now under progress. Priority setting may result from centralized, "top-down," problem-solving techniques that are not always appropriate for the district. A poll of healthcare professionals in South African hospitals revealed that administrators of hospitals were regarded as effective leaders if their leadership style encouraged engagement and problem-solving. An emphasis on a decentralised and egalitarian (equity-fostering) approach, where problem-solving is the duty of all health workers, is emphasized in a national quality assurance plan that includes PSBH.

Background: Anxiety is one of the commonest mental health challenges facing medical students. Levels of anxiety have not been estimated among medical students in Zambia. Objectives: To estimate the prevalence of anxiety disorders and its associated factors among pre-clinical students during the COVID-19 pandemic in the Copperbelt province of Zambia. Materials and Methods: A sample size of 398 students was sampled using a 1 in 2 systematic random sampling technique. The Zung's Self-rating Anxiety Scale (SAS) was used to determine levels of anxiety. Adjusted Odds Ratios (AOR) and their 95% Confidence Interval (CI) are reported.

ABSTRACT

This article is based on a recommendation from an ongoing pilot programme in Ghana, Kenya and Malawi that has reached more than 800,000 children since 2019, with key findings: (1) The vaccine introduction is feasible, improves health and saves lives, with good and equitable coverage of RTS,S seen through routine immunisation systems. This has occurred even in the context of the COVID-19 pandemic, (2) RTS,S has increased equity in access to malaria prevention, with data from the pilot programme showing that more than two-thirds of children in the three pilot programme countries, who do not sleep under a bednet, have benefitted from the RTS,S vaccine, (3) To date, more than 2.3 million doses of the vaccine have been administered in three African countries, with a favourable safety profile, (4) In areas where the vaccine has been introduced, there has been no decrease in the use of insecticide-treated nets, uptake of other childhood vaccinations, or health seeking behaviour for febrile illness, (5) A reduction of 30% in deadly severe malaria, even when introduced in areas where insecticide-treated nets are widely used and there is good access to diagnosis and treatment, and (6) Modelling estimates that the vaccine is cost-effective in areas of moderate to high malaria transmission. Therefore, this recommendation includes funding decisions from the global health community for broader rollout, and country decision-making on whether to adopt the vaccine as part of national malaria control strategies.

ABSTRACT

The artisanal fishery in Burkina Faso, already fragile, has been severely affected by government measures to contain the spread of COVID-19. Fishermen were the first actors to feel the impact of the crisis. This study was undertaken to assess the impact of the pandemic on their activities in two fisheries in Burkina Faso. To this end, a survey was conducted in the Samendeni and Sourou fisheries. A total of 86 fishermen were interviewed. The variables collected were: unemployment rate, reduction in fishing time, variation in fish prices, loss of income, inaccessibility of fishing equipment. The results reveal that 31.4% of the fishermen were unemployed, of which 41.2% were from Samendeni and 17.1% from Sourou. A reduction in fishing time of 1 to 6 months was observed in 53.49%. The perception of the fishermen on the catches shows that 2.32% have made gains in catches, 27.91% have not noticed a variation in their catches and 69.77% have noted a decrease in the quantities caught. They lost an average of 30.3% and 22.16% of their income in Samendeni and Sourou respectively. This study revealed the difficulties experienced by fishermen during the restrictive measures to curb COVID-19.

ABSTRACT

This study presents a designed ICT based extension service delivery system for the sugar industry of Eswatini. The model is an improvement of the current system and it presents a delivery system that is void of many limitations. This model emanates from findings of a survey which involved all smallholder sugarcane farmers (N=172) and their extension officers (N=17). The survey investigated how information and knowledge are currently managed within the sugar industry. Basically, the model revolves around the use of mobile phones to relay information among the sugar industry stakeholders in a timely, more organised, productive and cost-effective ways, without contravention of the COVID-19 pandemic protocols. Sugarcane stakeholders can now be able to exchange information using the model without having to meet physically, which is what most of the traditional approaches required. The exchange of information can be in a form of voiced, pre-recorded information in the form of texts, audio, or audio visuals. This would go a long way in enhancing smallholder farmer's productivity as it has the potential of empowering more rural sugarcane farmers with crucial information for improved productivity. The model has the potential to sustain itself as the participation of the stakeholders is promoted.

ABSTRACT

Background: In May 2020, the international non-governmental organization Cooperative for Assistance and Relief Everywhere (CARE) sought to enable health systems in Tanzania to address COVID-19 health response. CARE leveraged existing partnerships with the Ministry of Health, Community Development, Gender, Elderly and Children and the President's Office of Regional Administration and Local Government in the largest geographic region of Tanzania, Tabora Region. At the time, the government declared Tanzania to be free from COVID-19 and did not permit partners to implement COVID-19 programming. In this context, CARE implemented integrated health systems strengthening support for government response to not only COVID-19 but also other infectious diseases.

ABSTRACT

The coronavirus global pandemic now poses additional peril to the already burdened healthcare systems with inadequate funding in Africa. This paper attempts to present the funding patterns for healthcare in Nigeria, Congo, Tanzania, Egypt, and South Africa. It also explored investor opportunities for lowering OOPs and increasing investor profits. Data were drawn from the WHO, the World Bank, and world charts databases. Government health expenditure was far below what individuals and families pay from out-of-pocket payments (OOPs) for healthcare services except in South Africa, where OOP accounts for only 8%. Contrary to popular notion, donation funds or foreign aid for healthcare accounts for a lower percentage of total healthcare expenditure in the countries under consideration. Households mostly bore the healthcare expenditure burden (Nigeria's OOPs of 77%, Congo- 48%, and Egypt- 60%). Averagely, Nigeria and Congo only spent about 3% of their national budget on healthcare. In comparison, Tanzania and Egypt spent an average of 4% and 5% (South Africa-8%) between 2010 and 2017, reflecting low-risk protection for households. Specific aspects for investments through public-private partnerships (PPP) reviewed in this paper should include designing innovative financing models focusing on risk pooling mechanisms to help bridge the funding gap, local production, and manufacture of pharmaceuticals and healthcare equipment at this time, instead of importation. Investors can, therefore, take advantage of the various initiatives outlined in this paper to achieve better health outcomes in Africa.

ABSTRACT

Background: The COVID-19 pandemic lockdown resulted in the disruption of health services in Zimbabwe. The objectives of this study were two-fold. First, the study sought to determine the impact of the lockdown on maternal and perinatal outcomes at two tertiary hospitals in Harare, Zimbabwe, using a maternal audit. Second, the study sought to estimate the potential effect of COVID-19 related decreases in coverage of maternal and newborn health interventions on maternal, and neonatal mortality in Zimbabwe using the Lives Saved Tool (LiST).
